Amber Valley CVS – advertising recruitment to the Community Development Team ; Social Prescribing Development Worker x2 –re advertisement The main roles of the Social Prescribing Development worker will be: • To further develop, expand and deliver a Social Prescribing service in the areas of Alfreton, Ripley, Crich, Heanor that will link patients referred from GP surgeries in the ARCH Primary Care Network with appropriate services and support from the voluntary, community and non-profit sector. • To inform and support the development of new and additional services that would be beneficial to patients in Amber Valley • To prepare a plan with the patient and use motivational interview techniques to encourage personal responsibility in the outcomes with the patient. Salary per annum £19156. 80 + pension.Hours of work up to 30 hours per week, occasional evening work may be required Holidays 6wks,pro rata + bank holidays. Closing date: 9.00 am 28th October 20 Interview date: 12th Nov 20 Home from Hospital Project Worker This post requires you to develop and co-ordinate the Home from Hospital project with an emphasis on prevention of hospital admission. You will develop contacts with key referral points to gain clients. You will effective market for volunteers and be responsible for their training and match with those in need of either direct practical support or telephone support after discharge from hospital , or to prevent hospital admission. You will manage training, selection and placement of volunteers and be responsible for their ongoing motivation and support. You will need to be well organised, have good administrative and computer skills and be able to maintain records and statistics to monitor the project. You will also need to have empathy and good communication skills with people of all backgrounds, either those referred for support or those offering time as volunteers. Experience of networking with partners to promote and support project delivery is essential. Experience of supporting and developing volunteers is necessary with evidence of recent experience in delivering training. Ability to develop liaise and work with other agencies to meet needs of referrals is essential. Salary per annum £23636.30 + pension – pro rata Hours of work 20 hours per wk, occasional evening work may be required Holidays 6wks,pro rata + bank holidays.
